Paediatric microlaryngoscopy and bronchoscopy (MLB) is a commonly performed procedure in paediatric ENT departments. Thorough documentation of the pertinent intraoperative findings is essential to patient care, whilst a comprehensive clinical record is a critical facet of good clinical governance.

We audited the recording of intraoperative findings of MLB at Great Ormond Street Hospital for Children before and after the introduction of an MLB operative record pro-forma. The quality of documentation improved following the introduction of the pro-forma. This tool is recommended for use both by surgeons routinely undertaking this procedure but also by those who may perform it infrequently, perhaps under emergent circumstances.
